 Below are the new patient forms. Please fill out the New Patient Packet completely and bring that in with your first appointment. New patients will also want to view the Health Information Practices form, if you like you can print a copy for your records. We also include a PDF version of our Services and Fees page. NOTE: Please bring your insurance card(s), so that we can make a copy for billing purposes. If a referral from your Primary Care Provider is required please bring that with you or have it faxed to us at (425) 483-6068.
